CIS 236 Group Project
Peer Evaluation Form

A group project is a cooperative and coordinated endeavor on the part of several individuals, focused on a singular outcome.   The grade for this project is based on the instructor's evaluation of the group's collective effort in preparing the project according and presentation.

We also want to consider the contribution each group member made to the successful completion of the project. To do this, I need your honest and objective evaluation of each group member for several factors (listed below). Your evaluations are confidential to encourage your objective appraisal. 

Assign each group member (including yourself) points based on a 5-point scale*. Assign points based upon your assessment of each group member's overall contribution to the project, reflecting on the factors listed below. Please take your time and be thoughtful about how and to what degree each group member contributed. Feel free to attach additional comments you think are appropriate concerning group members' contributions to the completion of this project.

No grade will be given on this project for the team until I have received a completed evaluation form from each member of the group. The points earned in this evaluation will factor into the Project Team Participation portion of the grade.

FACTORS TO CONSIDER WHEN ASSIGNING POINTS:

A. A,P&P Attendance, Preparedness & Participation: The group member took an active role in helping to do the work (planning and execution).

B. CREATIVITY: The group member contributed good ideas to help the team to do the work.

C. DEPENDABILITY: The group member could be relied upon to what he/she was assigned to do or volunteered to do.

 D. QUALITY OF WORK: The group member contributed high quality work.

*5 Point Scale:

5 = outstanding, extra effort, and critical to success of project
4 = above average, willing able, successful, and made a special effort
3 = average, did the basic work successfully but no special effort
2 = willing, but not very successful
1 = did not contribute to this project even though a contribution was expected
